Sign the doctor’s order and our contract to enroll your patient for RPM by Health Connect.

Your patient must record vitals and use their provided devices for 16 days every 30 days for Medicare reimbursement.

The RPM liaison will monitor vitals daily and let you and the patient know if anything is out of range. They will also provide advice on how to improve vitals and their condition.
ELIGIBILITY

We would love to work with you to monitor and improve your chronic condition! To get started with RPM, you must:
- Be a Medicare beneficiary
- Have at least one chronic condition that needs monitoring.
These conditions include, but are not limited to: CHF, COPD, diabetes, kidney disease, hypertension, etc.
Applicable Medicare CPT Billing Codes.
- 99453: Initial set-up
- 99454: RPM device and software monitoring
- 99457: 20-minute patient coaching
- 99458: Additional 20-minute patient coaching
