Through RPM by Health Connect, you and your physician can see a more complete picture of you and your condition and empower you to be more proactive about your health.

Talk to your doctor about enrolling to RPM by Health Connect and getting in touch with us.

Record your vitals and use your provided devices for 16 days every 30 days for Medicare reimbursement.

Your RPM nurse will monitor your vitals daily and let you and your physician know if anything is out of range. They will also provide advice on how to improve your vitals.
ELIGIBILITY
Is RPM right for me?
We would love to work with you to monitor and improve your chronic condition! To get started with RPM, you must:
- Be a Medicare beneficiary
- Have at least one chronic condition that needs monitoring.
These conditions include, but are not limited to: CHF, COPD, diabetes, kidney disease, hypertension, etc.
